Prescription Refills

If you need a prescription refilled please contact your pharmacy and provide them the following information:

• Your full name
• Your physician’s name
• The name of the medication
• The current dosage
• Your current Insurance information

The pharmacy will contact our office to refill your prescription. You may check back with your pharmacy as to when your refill will be ready for pickup. If your refill cannot be authorized either the pharmacy or our office will contact you. We recommend you call 3-5 days in advance of when your refill is needed.
